My parents bought the pass including [Last name] [First Name] as requested in the agency online form, and thats the name on the voucher. But they have also middle name and mother last name.
So in the passport is [First Name] [Middle Name] on the name section and [Father last name] [Mother Last name] on the surename section. Would it be a problem?

In the Japan the family name is listed first. As such, John Smith would be written Smith, John.
In other words, all is as it should be and you're good to go.
