Im planning to travel to Hokkaido from Tokyo in Dec. Would like to confirm if i can use the JR pass to travel from tokyo to Hakodate & travel within the Hokkaido? And also i will be travel for 16 days in total, which options will be best to buy for the JR pass as i noted the pass only valid for 7, 14 or 21 days.

Yes, you can use the JR Pass all the way to and around Hokkaido without any additional cost.
I think the 14 days JR Pass will likely be the best option. Use that to cover your longer trips and you can then buy local tickets for the missing two days.

Just to double confirm if i can do the activation of JR pass on day 3 of my trip?so it can cover my transport till the end of my trip.
