Hi,
Nobody knows your interests better than you, so I suggest you look at some comprehensive websites that show what each places offers and choose what appeals to you. The best sources are the JNTO page, Japan Guide, and the official city websites which all have good tourist info.

If you are seeing both the Hakone area and the Fuji 5 Lakes area, then the Fuji Hakone Pass covers both. For getting around each city, local costs are not so bad. You could use an IC Card for convenience. Although in Tokyo there is a good cost effective 2 or 3 day subway pass.

For Day 11, you don't fly from KIX to Kyoto. There is a Haruka train which is quick and efficient. The Icoca & Haruka discount ticket may save you money.

I don't see enough rail travel to make any JR Pass pay off. Japan still takes traveler's checks at major banks and main post offices, but it can take up to 30 minutes to get them cashed. You might instead use your ATM card at a Japanese 7-11 or post office. If you do plan to use your ATM card in Japan, make sure your bank knows about it first so they don't suspect fraudulent usage and freeze your account. You can also see what Japanese money looks like here. Note that you can not use 1 or 5 yen coins in vending machines and phones. Japan uses 100 volts at 50 Hz for eastern Japan and 60 Hz for western Japan. If you're coming from Europe, Australia, etc, you will need a voltage converter for many devices. Others are made for multiple voltages though (see the bottom of your device). The plug is almost identical to the North American 2 prong type.

Good luck.
