Hi there,
The first step would be to take the Hokuriku Shinkansen from Toyama to Tokyo station. There you tranfer to the Hokkaido Shinkansen to Shin-Hakodate-Hokuto station.
Finally, take a ltd. express train from there to Lake Toya. The route is fully covered by the JR Pass and takes about 7 hours to complete.
