We arrive in Osaka on the 18th of June and want to start using the JR Pass on the 20th when we go to Hiroshima. Once you exchange the pass (activate it) do the days start counting or is it from the first day of usage as we want to make our reservations for the trains as soon as possible.

You can select your own "first day of use" when you exchange the pass. This can be any day within the next 30 days, so you do not have to start using the pass right away. The pass will then start counting down from the first day of use.
You can start making reservations once you have exchanged the pass, no further steps are needed.
