Which Rail Pass Should I get?

Lets take a look at your costs first:

1. Narita airport terminal 1 > Tokyo: ¥3,140 (62mins, 79km)
2. Tokyo > Kyoto: ¥13,020 (164mins, 513km)
3. Kyoto > Osaka: ¥540 (28mins, 42km)
4. Osaka > Tokyo: ¥13,550 (194mins, 556km)
5. Tokyo > Narita airport terminal 1: ¥3,140 (58mins, 79km)

JR ticket cost: ¥33,390 for 5 legs

This is a trip of 14 days, and a 14 day pass costs ¥45,100. This means you'll need to spend  another ¥11,710 on JR travel before the 14 day pass becomes cost effective over normal tickets (there is no other pass option that covers all the locations you list). You could easily make this up with a day trip to Nikko or Hiroshima for example, but it really depends on your plans. Also note that you can make good use of the JR Pass within Tokyo too which should yield more savings.

Yes I was thinking that I will definitely be adding some treks within my Japan Trip. Going on a day trip to Nikko would be a good idea.
