Going straight into your your answers:

1.) Yes the JR Pass covers the way from Narita Airport - Tokyo. You can ride the Narita Express completely for free and in style into the heart of Tokyo.

2.) The Subway system is run by a different company and does not accept the JR Pass. You can however ride all local JR lines in Tokyo which covers all major wards and stations.

3.) Yes all Shinkansen lines, including the Tokaido Shinkansen are covered! No worries here!

4.) The same applies in Osaka (and Kyoto) as in Tokyo, the subway lines are not covered by the JR Pass but again you can ride the local JR lines including the Osaka loop line that makes a loop trough Osaka and visits all the important place. Here is a map so you can get a feel of what is covered.
