I have bought my 7 day JR pass kinda early, 30th Sept 2015. Therefore the validation is 3 months which means it will be valid till 29th Dec 2015. I will be traveling to Japan on 26th Dec, and I intend to change on 28th Dec is it still possible to exchange the pass? Does exchanged pass be within the 3 months validity?

This is indeed cutting it close, if you count the 30th of Sept as first day of a 90 day period. Then the JR Pass can be exchanged until Sunday, December 27, 2015. Otherwise the JR Pass will expire and can no longer be used.
My advise would be exchange it on the 26th or 27th, you can select a 1st day of use during activation. This can be on a later date, up to 30 days after activation.
