I recommend using a 7 day JR Pass for your travels. This will cover between all the cities in the above itinerary, only for local travel around Fuji and in bigger cities is it that you still may have to buy other tickets.

There are no direct trains between Kyoto -Narita Airport and Kyoto - Nikko. Most of the time you will have to make a transfer at Tokyo station. To find the best routes, be sure to learn how to plan with Hyperdia. This way you can find all trains covered by the JRPass, route information and more.

There are different buses from Gotemba going around the Fuji area, be sure to specify first where you want to do. [Here is the route from Tokyo - Gotemba.

Is the JR pass valid for bullet trains also? Will I require a reservation for travel between Tokyo and Kyoto/ Tokyo and Nikko?

Yes it is valid on all bullet train lines (some train services, the Nozomi/Mizuho are not included).

Reservations are not required but I do recommend making them. It makes travel a lot more relaxed.
