Hello,
I have a US passport but on the ticket for 5day pass that was mailed to me says "UK" in the box that says "date and place of issue," is this correct or does it need to say US? Or is it because it's from JRPASS LTD 320 City Road, London?

That is perfectly fine! The location is simply the place of issue and does not affect the validity of the pass.
