Hi there,
1.) You can get on the Shinkansen to Kyoto from either Tokyo or Shinagawa stations. With the JR Pass you can use both the Hikari and Kodama Shinkansen to Kyoto.
2.) You would transfer trains at Tokyo station
3.) Yes.
4.) I am not sure what time that is in Japan but you should be fine as long as you get on the train from Haneda Airport before 18.00
