Making sure what pass to get

Hello! Wondering what pass we should get. Our travels will look like this.

Leave out of Hachinoe up north
Head to Tokyo, prob around Ueno station area?
Then we want to go to Tokyo Disney, team lab planets, and Sensō-ji/ Nitemon Gate area,
