When i exchange JR pass, i can reserve a seat, right?
what documents i have to prepare for making reservations?

can i write the detail as i shown below for the JR information desk when i making reservations?

Exp.:
2 Adult
Date: 2.3.16(sat)
Sapporo(JR) Station (08:40) to Eniwa Station (09:03)
~ JR Rapid Airport 84 for New Chitose Airport

Hi there,

Yes, that looks fine. The best is to have the date/time and places where you want to travel to. The more detail the better of course.

You can make reservations right away after arriving in Japan and exchanging the JR Pass.
