2 jan 2014 - from tokyo to kyoto
3 jan 2014 - from kyoto to tokyo
4 jan 2015 - tokyo
5 jan 2014 - tokyo narita
Please advise what type of train pass, bus ticket needed advance booking and reservation.

Hi Ube,
I think getting a 7 day JR Pass would work out best. You can use the pass to travel to Karuizawa, Kyoto and Tokyo.
Here's a little breakdown of ticket prices without the JR Pass.
Narita Airport - Karuizawa ¥ 8,380
Karuizawa - Tokyo ¥ 5,550
Tokyo - Kyoto ¥ 13,020
Kyoto - Tokyo ¥ 13,020
For a total of ¥ 39,970. The 7 day JR Pass is 28,300 so you would save at least ¥ 11,670. I would recommend making seat reservations when you arrive at Naria Airport for the rest of your itinerary. This because travel around new year can be very busy.
