JR Pass Middle Name...

I can confirm that this is not a problem. The JR Staff will check your voucher against your name in your passport but a middle name is not required. Nor is the name check as strict as it is with airlines, small differences may be overlooked.

So don't worry about it :)

One more question - when we fill out the second form to actually receive the pass upon exchanging the voucher in Japan, is it OK to supply the middle name there? I know that form has a place to enter your middle name - so will it be okay to put it down there, so that the middle name will show up on my actually JR Pass? Or does it have to match with my exchange voucher?

I don't think that it really matters. If you want to be sure, ask then person doing the exchange, he or she will after all have to verify the information but again don't worry too much a bout a middle name.

Yes, I'll just put it on the second form (since there's a space for a middle name - while the order form for my JR pass didn't even have a space for putting it) and if it's a problem I'll just show him that it's identical with my passport name!
