Yes you could get on the train and pay the fare difference at your final station.

Going back to Tokyo would be a bit different, as you first would have to buy a ticket until the station from where the JR Kanto Pass is valid.

Meaning going back from Nagano to Tokyo, we need to buy a ticket from Nagano to Saku-Daira then use the JR pass from Saku-Daira to Tokyo?

If it's so, then do we need to alight at Saku-Daira then board another train or we can just stay in the same train and proceed to Tokyo?

Yes you buy a ticket from Nagano to Saku-Daira. You can stay on the same train but may stay on the same train but possible you'll have to use a non reserved seat.
