Currency of pricing and ordering question

When ordering the JR Pass on this site, what currency is the pricing?

Also, we are travelling arriving nov 16 and departing December 15. Am I correct that I can order the pass in mid September for the old rates? I would also like to confirm that…

1) once ordered, the exchange voucher is mailed to me?

2). I have three months to exchange it?

3) I then have 30 days to activate it?

4). Can reservations be made online?

5) If ordering via Japan rail pass.net, I would have to activate within 30 days so it wouldn’t work for my trip?
