Had our first trip last year visited Tokyo Kyoto Nara Hiroshima and want to go back this year where would suggestions for the second trip? would still like a few nights in Tokyo, trip will be about 10-12 days and would if needed get 14 day rail pass
Would like to have a bit more "chilled" experiences, are the islands to far?
And what about the smaller cities

Hello misterB,
There are plenty of options, the JR Pass is valid of all JR lines nationwide, so no problem there. With 12 days you could head to the south and visit Kyushu and Shikoku. Both are amazing places to visit and are a nice change for the busier city life found in Tokyo/Osaka.
