We have a 7 days to stay in Japan and our inbound and outbound flight is in Kansai International Airport.
I'm trying to understand that I can use the nationwide JR pass to get us from Osaka to Tokyo?
10/14 - 10/16 - Osaka
10/16 - 10/18 - Kyoto
10/18 - 10/21 - Tokyo
10/21 - Osaka
Is it possible for us to get around with this itinerary?

Yes, the JR Pass can be used for bullet train travel from Osaka to Kyoto and Tokyo (and of course the other way around as well).
The price of a 7 day JR Pass is about the same as a normal return ticket Tokyo - Osaka and is certainly worth considering if you cover any other travel with it.

How about if I go to tokyo on 10/14 then be back to Osaka (KIX) on 10/21? Its 8 days? but the JR pas is just 7 days?

You are right that the 7 day JR Pass would not work for a return 10/14 to to/21/
If for example,
10/14 - 10/17 - Tokyo
10/17 - 10/19 - Kyoto
10/19 - 10/21 - Osaka
On our last day, if the JR pass will not work? How could we use the JR to get to Kansai International Airport and how much would it cost us as an additional ticket? If we are coming from Shin-Osaka?

A one way Shin-Osaka to Kansai Airport is 1,160yen.
The key to using the JR Pass is to make use of it on longer routes, such as Tokyo - Kyoto.
