Sir,
We are visiting Japan from 22nd may onwards- family of 4. Staying in Narita.
Want to visit mt.Fuji - same day return,Disneyland and Tokyo city .
Need help with train travel.
What is the best travel pass which allow us these trips in 3 days - may be a three day pass.ex Narita airport.
Cost and which trains to take and itinery etc for the same
Any reservations required.

Hi Manoj,
I think you a JR Kanto Pass would be well suited for your travel. It allows for travel to Fuji, Narita and the Tokyo area (including Disney Land).
You can also find the best itinerary to travel on the website liked above.
The best way to travel to Fuji san from Narita is to travel to Otsuki and from there take the Fujikyu line directly there.
The pass should cover all of your travel costs. You can make seat reservations as you like but this is not required.
