Does the JR pass include the bullet train? Also, we are only in Tokyo for three days, would it be cost effective to buy a seven day pass? I assume the pass is for travel around the city.

Yes, the JR Pass covers all bullet train lines. It depends on where and how much you travel whether a JR Pass will help you make savings.
For instance, going to Kyoto would quickly make the JR Pass a great option, while just staying local in Tokyo would not.
