Trip to Japan for 33 days

Hi, I plan on going to Japan for a little over a month and I just needed to know which jr pass would best fit my itinerary. I'm looking at the 21 day and 14 day passes. So here's what I have so far:
(This is a very generalized schedule)

May 13-May 28: Tokyo with day trips to Nikko, Nara, and Disney Resort
May 28-May 31: Kyoto
June 1-2: Osaka
June 3-5: Hiroshima and Miyajima
June 6-15: Tokyo with one day trip to Mount Fuji

It might sound all over the place but this is the best schedule I could work around.
Also, do I possibly need to buy 2 separate rail passes to make these trips possible? And what would be the most cost effective time to activate the pass to begin using it? Thanks to all who answer!

Hello there,

When considering the above itinerary, I'd opt for using a 14 day JR Pass can cover the following travel with it.

Tokyo - Nikko (return)
Tokyo - Kyoto/Osaka
Osaka - Hiroshima
Hiroshima/Miyajima - Tokyo

This would cover all pricey long distance travel and you'd make good savings over normal tickets at the same time.

It is also possible to buy 2 separate passes but that will cost more than a single 14 day pass.
