Would love to know if you think a JR pass would be appropriate for the following itinerary?

Arrive Tokyo Narita Airport & stay in Tokyo for 5 nights
Leave Tokyo for Kyoto & stay in Kyoto for 5 nights
Leave Tokyo for Osaka & stay in Osaka for 3 nights
Leaving Japan via Osaka airport

If a JR pass is suitable, do you think a 7 or 14 day pass?

We plan to get the limousine bus from Narita airport to our hotel in Tokyo because it will take us to the hotel directly. Do you know if limousine buses need to be booked in advance?

Answering some of the questions:
1. Depending on the time and the number of luggage, it is possible to use the JR pass from Narita to Tokyo Central station and take a taxi. It's faster.
2. In Tokyo would use the pass to go to Nikko and near Mount Fuji
3. In Osaka, would use the pass to go to Nara, Hiroshima and Himeji
4. Kyoto is far one Shinkansen station to Osaka.

Unfortunately a 7 or 14 day JR Pass would not pay off for your itinerary. You'd have to extend your trip some, but that is not difficult to do. A day trip to Nikko would be well worth it. Also because you are already seeing Tokyo, you can largely skip Osaka during the day. After the temples of Kyoto close down around 5PM, you can zip over to Osaka for the evening. At night the city comes alive and has some great places to see, such as Dotonbori, plus the night views from the Umeda Sky Bldg, and Abeno Harukas Bldg are wonderful. Instead, you could go to Hiroshima/Miyajima for a night, which would make a 7 day pass worth it. You could also visit good sights nearby Kyoto, including Nara, Koyasan, and Himeji.
If you don't want to get the full JR Pass, then look at some regional passes to save money:

Instead of the expensive limousine bus, look into the cheap 1000 yen bus from Narita or the Keisei with a Metro Ticket:
