Hi,
I highly recommend you see Hakodate - especially Mount Hakodate and Fort Goryokaku. Going all the way to Sapporo would take over 8 hours and be extremely tiring. It's up to you to decide to overnight there or not, although the view from Mt. Hakodate is particularly beautiful at night.

Within Tokyo, you can use a 2 or 3 day subway pass, and the cheapest bus to/from Narita Airport is the 1000 yen bus.

There are also some good [passes for Yokohama]. Whatever you do, don't miss the Sankeien Garden there.
