I'm looking to travel to all these places when I visist Japan.

Is there one rail ticket I can buy that will cover all these places?

The only rail ticket that will cover all these locations is the Nationwide Japan Rail Pass which you can purchase here. To check the savings with a JR Pass, here's the cost breakdown for your trip with comparisons with the savings you'd make by purchasing the various lengths of JR Pass:

Tokyo > Osaka: ¥13950, (194mins, 556km)
Osaka > Hiroshima: ¥10150, (104mins, 345km)
Hiroshima > Tokyo: ¥18250, (292mins, 894km)
Tokyo > Saga: ¥23670, (419mins, 1228km)

JR ticket cost: ¥66020 for 4 legs

07 day rail pass (¥28,300) saving: ¥37720
14 day rail pass (¥45,100) saving: ¥20920
21 day rail pass (¥57,700) saving: ¥8320

You do have a very long trip going back to Saga from Tokyo, if possible I recommend you visit Saga after Hiroshima.

Note about non-JR trains and metro systems

Please note that the Japan Rail Pass covers trains operated by Japan Rail (JR). This includes the Shinkansen bullet trains and the vast majority of express/local trains however does not cover municipal operated subway and metro systems, which will require additional ticket purchase.
